We ask that ALL students complete the following 3 forms, found in the folder titled “Handbook & Forms for ALL Students – REQUIRED”:
- Handbook form
- Extracurricular form
- Medical Release form (“Authorization and Release for Travel”)
These forms help ensure the safety and understanding of all of our students at all choir events both on and off campus.

Choir Trip Forms
If you will be attending the trip, necessary forms can be found here. This folder will be updated with the necessary forms once trip details have been determined and shared in class.
Voice Lesson Application
If you would like to take voice lessons at BHS once a week from our amazing private voice teacher Kristin Chaudhary, please fill out the form and return it asap to your director. The studio fills up quickly on a first-come-first-served basis. Please look at the form for more details.
Honor Choir Application
If you are in 4th period A Cappella choir, you may choose to enroll in Honor Choir. Students in Honor Choir are responsible for additional work including a theory workbook, concert reviews, and research papers, and they earn honors credit for completing it. Honor Choir is a year-long commitment.
